It's Open Enrollment time again! This is the only time of year you may elect to make a change in your healthcare plan. Now is the ideal time to take stock of your current health care plan and decide if you'd like to make a change to an alternate plan, or to keep your current health insurance coverage.
Each person and family has different needs. If for some reason you feel another health plan might be a better "fit" for your needs, now is the only time of year you may make a change without having to meet the criteria of a "qualifying life event" (i.e., marriage, divorce, change in spouse's employment/benefit options).
The 2019-2020 Health Benefits Expo will be held Wednesday, May 8, 2019 at the Educational Services Center, 16 Welden Drive from 3:00 - 6:00 pm.
Below you will find plan highlights for our benefit plans including Medical and RX. To make a change to your medical plan, you must contact Human Resources at 267-893-2010 BEFORE June 30, 2019.
If you are not making any changes to your current medical plan, no further action is required on your part.

1. CBSD Open Choice 1 (10/20/70): PPO Plan allows in-network services with a co-pay $10/$20/70% reimbursement (deductible may apply). Click HERE.
2. CBSD Open Choice 2 (20/40/70): PPO Plan allows in-network services with a co-pay $20/$40/70% reimbursement (deductible may apply). Click HERE.
3. CBSD Open Choice 3 (25/50/50): PPO Plan with in-network deductible for specified services ($1,00 single/$2,200 family). Specific services with co-pay (25/50). Click HERE.
4. CBSD Point-of-Service - POS Plan allows you to stay in-network (requiring referral) with co-pay ($15/$25) or you can self-refer in/out-of-network, however, higher out of pocket costs may apply. Click HERE.
