If you have not already done so, please make an appointment to have this obligation met as soon as possible. If your student is participating in sports this year, that physical will be accepted.
As you take your child to the doctor for working papers and/or driving physicals; please remember that a copy is needed for school. This is a state requirement and may be accepted in 10th or 11th grade. The official form is available in the nurse’s office or on the district website. If this requirement is not met by the end of the school year, it will become an obligation and will result in loss of privileges and the holding of your student's diploma. If you have questions about the physical examination, please contact Mrs. Carol Klein in the Health Office. For more information, please review the information on the CBW Health Office Website: https://www.cbsd.org/Page/6310
