State law requires that all students submit documentation of a physical exam in 11th grade. (Public School Code Article XIV, section 1407.) Documentation of a physical exam completed at any time within one year prior to entering 11th grade will meet this requirement.  Therefore forms submitted during 10th grade are acceptable. If your child has a PIAA physical on-file, there is no need to submit a physical to the health office. 

                   Physical exam forms are due in the health office by October 15th
