One that facilitates and promotes optimal learning by promoting good health among students and staff.Welcome StudentsAll 7th Grade students MUST have the following Vaccines by September 6th 2022:- Meningococcol (MCV)- 1 dose
- Tetanus-Diphtheria-Acellular Pertussis (Tdap)- 1 dose
All 7th Grade Students MUST have an Updated Dental Form by October 15th 2022
All New students to the District entering 7th, 8th, or 9th Grade MUST have the Following:
- Updated Dental & Physical Forms- See below for Forms
- Meningococcal Vaccine (MCV)- 1 dose
- Tetanus-Diphtheria-Acellular Pertussis (Tdap)- 1 Dose
- All age appropriate Vaccines Required by Pennsylvania state law
- Medical forms
- Immunization Regulations
Any Students that require medication in school MUST have a Physician's Order- even if the Student is to Self-Carry (Inhalers/Epi Pens). Both a parent/guardian and a physician MUST sign the form.
**Students are NOT allowed to administer any medication to themselves while at school. Please contact the Health Office with any questions or concerns.
****Parents/Guardians- Please complete the online Census form via Parent Portal Prior to the first day of School***
