The state of Pennsylvania requires physical and dental exams upon initial entry to school. 

 

Dental exams are required again in 3rd grade.

 

Physical exams are required again in 6th grade.

 

Please submit the completed forms to the health room or email them to cbkutznurses@cbsd.org
