• teddy_bear_nurse Health Info./Nurse

We are very fortunate to have a wonderful nursing staff here at Groveland. I provide band-aids for minor cuts and scrapes. Most of the time, students will only be sent to the nurse if they are ill, need to take medication or if they have an accident or injury.

Each year, the Central Bucks School District asks our families to fill out a Student Emergency Information Form. This form ensures that student information as well as your contact information is accurate in case of illness, injury, or emergency. This form must be completed online each year.

If you do not have a Parent Portal, go to https://www.cbsd.org/Page/26932 to set this up. The Parent Portal is a valuable tool that allows parents to view schedules, attendance, immunizations, and assessment information, as well as update demographic information for their family.

Once you have a Parent Portal account, go to:  https://www.cbsd.org/Page/26952 to complete the Student Emergency Information Form.

In the Parent Portal please choose the link in your message inbox that says “Student Emergency Form”.

Next – “Click here to update an Existing CB Student Emergency Form”

It will ask you to select a language (English or Spanish).

It will now take you to the Infinite Campus Online Registration – select the “Begin” button and from there you will be able to complete this information

After you hit the “SUBMIT” button, you will receive an email thanking you for completing.

Health Forms - Please make sure you have turned in a current physical and dental form to the health office.  Here are the necessary forms.  They can also be found on the CBSD website or you may request a form to be sent home.  Please know that children without state mandated exams on file are not permitted on field trips.

Health Concerns – Please communicate any medical conditions or health concerns your child has to our school nurse. Also, please communicate any communicable illness your child has including, but not limited to: strep throat, Fifth’s disease, pink eye, lice, etc. Please review with your child the importance of good hand washing after using the bathroom, before eating and keeping fingers away from their "T zone" (eyes, nose and mouth).
