Unami Sports Physical Policy
Pennsylvania
Interscholastic Athletic Association (PIAA) regulations require a
physical examination each year and parental consent before a student can
train or practice for a sport.
Sport Physical Forms
are available in the office or can be downloaded below.
If a student participates
in several sports during the same school year, it is not necessary to
get another complete physical. The school doctor or your own
doctor can recertify the student after an examination and review of the
student’s health history checklist prior to the start of the new sports
season.
