Household Membership Changes

All student household membership additions, deletions or changes must be documented on the Household Membership Change form. This would include the following:

Parent/Guardian changes (Additions/Deletions of Household Members)

Sibling changes (Additions/Deletions of Household Members)

Mailing changes

Student Household Membership Change Procedures:

  1. Obtain the form from the school your student currently attends
  2. Complete the form and return to your student’s school
